How to appear in Google AI Overviews

Google AI Overviews is the most established but most conservative of the four major AI search surfaces. Appearing in an Overview citation requires foundational SEO work plus AI-specific structured data. This guide walks through the engineering protocol.

Google AI Overviews appears at the top of Google search results for an increasing share of queries (estimated 30-40 percent of all US queries by mid-2026). The Overview synthesizes an answer from multiple sources and lists them as cited domains in a sidebar. For specialty practices, being cited in an Overview is high-value because the Overview occupies the most visible real estate on the search results page.

1. Allow Google-Extended in robots.txt

Google-Extended is a separate crawler from Googlebot, controlling whether your content is used in Gemini and AI Overviews. Many sites block it by default through privacy plugins. Add this to robots.txt:

Allow: /

2. Rank in the top 10 organic results

AI Overviews draws candidates from the standard Google SERP. If you do not rank in the top 10 for the target query, you cannot be cited in the Overview for that query. Traditional SEO foundations matter: backlinks, topical depth, page speed.

3. Ship complete Schema.org structured data

Google AI Overviews relies on structured data more than any other AI surface because the parent Google search index already prioritizes structured signals. MedicalClinic, Physician with hasCredential, MedicalProcedure, Drug, Offer, and FAQPage are all directly relevant for medical practices.

4. Satisfy E-E-A-T trust signals

For YMYL queries (Your Money or Your Life: medical, legal, financial), Google applies aggressive trust filtering. E-E-A-T stands for Experience, Expertise, Authoritativeness, Trustworthiness. Practical implications: author bylines on every article, full credentials on provider pages, accreditation badges from recognized industry bodies, and clear policy pages (privacy, terms, editorial standards).

5. Use answer paragraph format on every page

The same Answer Capsule pattern that works for ChatGPT works for Google AI Overviews. 40-60 word direct answer at the top of every page, fact-dense, named entities, specific facts.

6. Submit sitemap via Google Search Console

Sitemap submission via Google Search Console is the standard path. IndexNow does not affect Google directly but submitting via GSC plus IndexNow to Bing covers both surfaces.

Timing expectation

Of the four major AI search engines, Google AI Overviews is the slowest to cite a newly rebuilt site. First Overviews citations typically appear day 35 to 50 after launch.
